Abstract Background Lingual edema is usually the result of infectious etiology, angioedema, and less commonly traumatic etiology. As for therapeutic approach, first priority excluding airway compromise or ensuring safety. Antibiotic treatment should be administered tongue swelling inflammatory etiology while standard therapy with epinephrine, steroids, antihistamines recommended cases anaphylaxis. In addition, in case injury causing lingual edema, one options surgical intervention. Aim work The aim our to present a rare simultaneously instructive hemorrhagic tongue, leading obstruction. Case presentation We report initially without symptoms upper obstruction on admission. However, immediate intervention securing was necessary due rapid growth edema. Then, as long patient hemodynamically stable patency ensured, diagnosis iatrogenic hematoma confirmed. challenge decide appropriate patient. Following, she taken 24 h ICU lactic acidosis bleeding treated conservatively. Afterward, transferred clinic, stable, discharged uneventfully. Conclusions Hemorrhagic may become life-threatening condition. Its can lead respiratory way; therefore, special wariness its management required. Treatment conservative surgical, depending size tendency increase.
